You dont need to provide medical receipts, there is a form sent to u in the letter where u will fill up the name of the panel physician examined u and their address along with ur application number. This is enough. I hope this info helps :)

Thanks a lot...yeah got that one filled up by prescription point. They put the doc's name stamp and IOM's address stamp on it :)

So there is a bit of confusion regarding the four photographs to be sent.

On the letter that has the list of documents to be sent it says: "on the back of only one photo, write the name and DOB of the person in the photo"

Then below that line there is a link for photo specifications for PR card where it says to write the date the photo was taken behind the photos.

But at the end of the letter where there is a separate photo specification and instructions...over there it clearly says in bold "do not write anything behind the photos"

Now i am super confused...which instruction should i follow? ??? ??? ???

Apu, photo specification is a generic one. Its says how to take a picture and measurements bla bla.

But your instruction letter is more specific, they asked for 4 copy picture and in 1 copy you need to do this and that. I think we should go with the instruction letter.

U r scaring me out now, i just followed the instruction and wrote name amd dob on one photo. But did not go to any link and did not know that the date of the photo taken must be written. Should I b worried?? I dont know, already sent it. How sgvo will look at it.

Just name & dob on one photograph. That's it. The instruction for PR card doesn't apply here.

Don't worry bro. I forgot to write date of photo taken as well. But there was no issues.
Although my visa office is different. But I expect you won't have issues as well.
 
Thanks everyone for the replies. We decided to send some extra photographs (with nothing written on the back). I don't think they'll reject our application only because we are sending them some extra photographs :P :P :P
