folks, i have been asked to submit updated IMM 5669 Schedule of background/ declaration form, along with PCC, PP etc.

can anyone post the link of the latest version?

thanks a lot !

Here it is vaia...:

Schedule A – Background/Declaration [IMM 5669] (PDF, 246 KB) December 2012

http://www.cic.gc.ca/english/pdf/kits/forms/IMM5669E.pdf

know it might sound redundant and stupid, however, just wanted to be reconfirmed about the below issues of RPRF:

1. Bank draft can be either SGD or CAD, right?
2. Does Standard Chartered Bank in dhaka issue bank draft in Canadian Dollar?
3. for me and my spouse, there will be only bank draft for the total amount of CAD$ 980 ($490 for each).
4. for my dependant child aged 5, i dont need to pay anything
5. Shall i submit the bank draft to the VFS global, Dhaka? (in the letter, they mentioned "you must submit the fee directly to the High Commission of Canada in Singapore".

Thanks a lot for bearing with stupid clarifications.
regards,

thanks Ashif ! you are a gem.

i have kept my fingers crossed for your good news; i strongly believe today you are gonna get it.

For query 1,2,3 its a yes
Query 4 I'm not sure
Query 5 its my questiom too. Bcoz in the letter they mentioned to submit all docs with the passport. So should i submit pcc, schedule A, RPRF fee cheque from SCB, and other docs at canada visa application centre along with passport???

One more thing, you know the Appendix A, they said it needed to be filled up by spouse as well, does it mean we shall send over two of these forms one filled up by me and the other by my wife?? Doesnt make sense to me. It wud be the same information.

for appendix A, i will just just take two printout and send.... to be safe...

and, did you also get 30 days timeframe for submitting?

did you do medical already with wahab?

thanks.

did you make your bankdraft already? from Standard chartered?

hi Ashif:

Thanks for your help since the inception.

1. i decided to go for manual payment (bank draft) rather than online to avoid any future problem.
2. i sent CSE two days ago about my wife's cali state report, and they replied that still require that, and within 30 days ! bugger ! ( i strongly suggest you to get bhabi's state report as well to avoid future delay)
3. i have done medical already.
4. i have asked for passport also to submit with docs WITHIN 30 days. what i have decided is to submit all docs except passport and cali state certificate, and would mention that i wud submit these pending two once i get cali state report.
5. as they asked for 4 photos, shall i send the photo with the first batch of docs which includes all docs except Passport and cali report?

any comment on above?

finally, i believe you are in our prayer, so, i guess this saturday you will get all.

thanks.
Boss whats up with the cali pcc?? Im quering bcoz my uae pcc is asked by sgvo even though i left the country before turning to the age of 18. Wondering should i still process that. Thanks

Dear mates, any idea why we are skipping? How many days we needed to be patient :'(

Brother you were right on your Query number 4 dependent children not need RPRF fees.

One more thing, in the letter sent by Singapore visa office regarding the background declaration form did They mention whether to fill up The personal history and addresses from the age of 18 or upto last 10 years???

at the time of application, i gave last 10 years, and now, i am giving since age of 18 which is last 16 years as they mentioned in my letter that it has to be since the age of 18 (not last 10 years only)...

over info is better than shorter info... :)

cali pcc means police clearance of the state of california, USA...

i sent them FBI clearance... but, then the asked for California specific PCC last week... and then i emailed them requesting to waive, but, they replied, they still require that.... i am going to apply for California PCC this monday ... i will submit all my docs except cali pcc and passport and will write them a letter asking for more days to submit cali pcc....

in your case, i would strongly suggest to send them CSE about your UAE PCC.... btw, i also sent my UAE PCC to them... i had to submit PCC of 5 countries ! lol

good luck !

Hi,
Can the bank draft for RPRF be done at any branch of SCB? Also, what are the associated documents needed for issuing the bank draft from SCB? Thanks for your help in advance!
