FSW 2013 Documents: "HOWTO complete" questions

I am currently trying to complete the "Generic Application Form for Canada [IMM 0008] (PDF, 536 KB)" form:

Question 7 indicates the following:

Have you received your Certificat de Sélection du Québec (CSQ)?


But I notice there is no option for writing "N/A" (since I am not applying to live/work in Quebec), The only option for the question is "Yes" or "No".


The Application for permanent residence: Federal skilled worker program (IMM 7000) document doesn't really help to clarify what I should indicate if the question is "not applicable".

a. Check the corresponding box
b. If you checked “Yes”, please indicate the CSQ number
c. If you checked “No”, indicate the date when you applied for your CSQ

Note: If you are applying for refugee protection, check “no” for Question 7a and leave 7c blank.

It is an automated form, so if you complete it electronically it should "grey out" the yes and no answers so that it becomes more obvious that they should not be completed.

You are exactly right about the form "greying out" for that question (and some of the other questions).

It's strange, I noticed the "grey out" for the other questions but couldn't see the "grey out" for that question. :-[

Also, on the Document Checklist [IMM 5612] (PDF, 233 KB) form, item 9 indicates original IELTS test results is to be submitted.

My IELTS results came with what appears to be 3 original result certificates, 2 of the certificates are colored green (with the text written in black)
while the 3rd result certificate is colored white (with the text written in black).

All 3 result certificate seem to be exactly the same in terms of content, its just that two of them are the same colour (green) while the other is white.

Which result certificate is CIC expecting me to send (the green or the white)?

I Wrote N/A with a black pen on my application to avoid blank field. I sent my application last week.

I am not a expert.

I'm not an expert either, I pondered 3b, 7c and 9b on Schedule 3 also and just ticked the No boxes in the end. In hindsight I suspect writing N/A next to them would be better (if less tidy/automated) as it is less ambiguous for them, though I doubt it causes a great problem either way.

My IELTS certificate was provided only in Green.

I did some checking at CIC's site, and this is what they have stated regarding completion of the forms:



How do I fill out an application?

Here are a few tips for completing your application form.

Answer every question, even if it does not apply to your situation. If an answer or the whole application seems incomplete, the application may be returned or refused.

If you do not have an answer to a question, and there are no instructions for it on the form or in the Instruction Guide, write “not applicable” or “N/A” in the space provided. If necessary, briefly explain why it does not apply to you. You must write something in every space on the application form. That shows you have read and filled out the form fully.

If your answer to a question will not fit in the space on a printed form, write your full answer on a separate sheet of paper. When doing so, you must note (on the paper) the exact form you are using and the letter or number of the question you are answering.

If you would like to provide more information such as details of your circumstances, please do so on a separate sheet of paper.

It would seem to me that it would have been so much easier for CIC simply place a "N/A" option for each question on the form.

guys, just wan to know your opinons on this:

is it ok to send my spouse's forms (just the pages with his signatures) scanned copy only? he's working in another country right now. is it acceptable to have an explanation as to why they are scanned copies and originals can be sent if CIC requests for them? my spouse is not the principal applicant.

According to page 2 of the Document Checklist [IMM 5612] (PDF, 233 KB), you are expected to send the forms in "Original" format.

I do not know if it will be ok to send scanned copies of the signature page (even if you send an explanation page).

Since he is your spouse, couldn't you just sign his signature on the form (I guess technically/legally it might be wrong to sign for him though).

For the Mailing Your Application section (item 21 on page 7) of the Document Checklist [IMM 5612] (PDF, 233 KB) form, it says :

include 2 self-addressed mailing labels:
- one in English or French and
- one in the official language of your country of residence

I am not sure what exactly is that "self-addressed mailing label"?

I intend to use one of the usual courier services, namely FedX, DHL or UPS.

Do I simply tell the FedX/DHL/UPS agent that I also want to purchase a "self-addressed mailing label" to insert into my courier envelope?

It is just so they can stick something on the front of an ordinary letter. They are not wanting you to prepay any postage or anything.
Just write your name and address on a couple of plain sticky labels and you are good to go!
