I did that for two of my employers;
Company 1) - Did not provide me any letter; not even bothered replying on email. So I attached notarized affidavit and attested copies of gmail where I was continually requesting for such letter.
Company 2) - They have their own format and offered me a letter for visa addressed to CIO, NS. They did not mention job roles. So I attached notarized affidavit and referred to company's letter number on the affidavit.

Although, I've not got a PER but CC's already charged.

My NOC's 0211 and app received by D.Margetti. I got my CC charged after 76 days (two and a half months) of filing the application (31st-May) and yet to hear anything about PER. Since, your app received on 3-Jul you have a long to way to go. Probably, you can hear any news after approx. 60 to 80 days starting from 3rd July.

Currently, CIO is working on apps received in first week of June so your app is still lying unopened in the queue.
