I am not sure what exactly is that "self-addressed mailing label". I intend to use one of the usual courier services, namely FedX, DHL or UPS.
Do I simply tell the FedX/DHL/UPS agent that I also want to purchase a "self-addressed mailing label" to insert into my courier envelope?
"Self Addressed Mailing Label" means your own address. In case if CIC want to contact you they can simply stick the "Self Addressed Mailing Label" on the envelope and send it to you.
You may include two one in English and the other in your regional language which the postman or the courier boy will understand. If english is one of the official languages of your country then include both the labels in English.
