Form IMM 5669 - Confusion

As soon as I open form IMM 5669, Question 1 and 2 ask for UCI number and Application Number and then to the right it says in a box Office Use Only - Validated

Do I have to answer these 2 questions or do I leave them blank?

I know my UCI number...but what is my application number?

Please advise

Your app starts with the letter F. Will be in your aor

If you're including IMM 5669 as part of your initial spousal sponsorship application, you won't have an application number and can leave that space blank.

I have my UCI number because ever since I came to Canada back in 2005 as a student I have had the same UCI number which was on my student permit
