Fingerprint form details for Mexico Police Clearance

Hi All,

What is the name of the finger print card we need to send to Mexico for PCC. For USA FBI PCC, I used the FD-258 form, but FD-258 specifically has FBI printed on it, which means it applies only for USA maybe.
Can someone help me with the fingerprint form type/name that we need to use for sending to Mexico ?

Hi all
Please help me and advise how to do with Police check mexico .
I’m Vietnamese
I have been living in Mexico and now i applied visa for Canada , is request to do PCC mexico .
i did the Fingerprinted by FBI form end send to mexico . they send its back and Said :My Name in not correct in the form as Passport .
for example :
FBI form :
Last name : Nguyen
First name : Huong
Middle name : Tuyet
But my Passport name :
Full name : Nguyen Tuyet Huong
Which is Inverse and they not accepted it .
I’m so stress out and can’t find the way . i’m keep finding how to do .
they make me sended 2 time and nothings work
Please show me which form i can do correctly .
