Filling out IMM 0008 Form to sponsor spouse from outside Canada

Hello:
I am in the processing of preparing the sponsorship application to sponsor my wife. She is from the Philippines (Manila). In the IMM 0008 form, Under the section: Application details: Question #4 : immigration office requested to process this application: Do we answer and type Manila office or the Processing Center in Mississauga

Under the section: personal details: Question #1: Full name - the family name: Do we answer and type my wife's married name or is it her family name.

Also Under the section: personal details: Question #3: UCI: we do not have UCI, do we leave it blank or do we answer and type N/A

I would really appreciate it if some one can help me answer the above questions. Take care and God Bless

We put "Canadian Embassy - Philippines".
Put the name as it appears on her "passport".
If the form will validate with "N/A" go ahead and try that, otherwise leave it blank and you could write N/A on it after printing.
