in my case, i included my middle name because it was specifically stated "as shown in your passport or travel document". In item #1 (page 1) of the Application for PR form, I wrote my first names and middle name side by side on the space for "Given name(s)". In page 2, "details of family members", I also did the same for my husband and son.

In Schedule 1, item#1, I placed my middle name on the "blank" field below the "Given name(s)". In items #7 and #8, I also placed the middle names of my parents on the blank fields below the "Given name(s)".

I also did the same on Schedule #3.

BUT on the Additional Family Information forms, I wrote the names (mine, spouse, dependent child, parents, and siblings) with the MIDDLE INITIAL ONLY, starting with the first names and ending on the last names.

I guess now, if a choice which to follow. I think they (CIO) is not going to be strict if a middle name is included. Just a hunch on my end.

what I used in US is magicjack... pwede sya US & Canada. You have to buy 2 units of magicjack one for you & yun sa pinas na lagi mong tatawagan. You can use magicjack via internet for just $20/yr. about sa roaming globe or sun nman...gamit ko sya for incoming text lang kse mahal kung gagamitin for outgoing call/text & accepting calls. You can use chikka text just download it..its a free sms. I dunno about some other promos of globe for international call pero mura pa rin ang magicjack for unlimited call.

also, dun sa work experience requirement, nakalagay dun na include a business card of the signatory. what if kung walang business card yung pumirma o kaya hindi naman updated yung business card nya, ok lang kaya na di na ako magsubmit nun?

photocopy yung passport and employment contracts namin and notarized (please follow kung ano dapat nakalagay don sa papa notarized mo kase may intruction ang VO don). pero ang reference letter or employment certificate dapat original.
kung di ka makakuha ng business card ng employer mo, make sure lang na ung reference letter mo ay may company name and address/tel no kase yon na rin ang purpose ng business card. palagyan mo na rin lang ng contact number ng signatory para alam nilang pwede syang tawagan anytime.
yung isa pa nga na dapat may logo ng company, yung isang employer ko wala silang logo so naglagay na lang sya ng note sa baba na walang available logo yung company.

Medyo na confuse lang ako sa Nurse Educator and Nurse Instructor because I find the description synonymous. Even Clinical Instructor parang the same lang din ang job description nya but then I have second thoughts regarding CI kasi Instructors were removed from the list of new job categories.

i read your case.... yung work experience mo ba nasa NOC for FSW pa din until now? if yes, if i were you, this is not an advice, opinion ko lang, i would withdraw my applications (FSW 2008 & quebec)... then file another application under the new rules etc... baka abutin ka ng apat hanggang anim na taon.... wag ka na rin kumuha ng agency, dito ka na lang sa forum... ako hindi ako nag agency, yung nagyaya sakin na mag apply anjan pa hanggang ngayun.... anyway, opinion lang you can ask nars... he's an expert...
