You should only fill out the top part of the form because you paid online. You need to attach the receipt though.
Are you referring to page 4, the oath?hisaca said:Hello,
There is a declaration portion at the end of IMM 5669, background form. We are told to leave it and not sign it yet.
Is that correct, can we leave it unsigned?

Thanks for the reply. Would you please kindly share your application timeline?
Mine is as follows:
Application received: Feb 8, 2014
Medical request: March 1, 2014
Medical received: April 25, 2014
Application status: Application received
RPRF: paid with application submission
RCMP: not submitted or requested yet
IN process: ???
Decision made: ???
PPR: ???
Landed: Inland
Do they request RCMP for all Inland applicants or is it selective???? Didn't know about RCMP request at all.

HI if you have never applied for any CIC related document like study permit or work permit or any other permit, you will not have a client ID or unique client identification number (UCI). I don't think you need to be worried if this is the case. But in case you have applied for any document from CIC the document should contain this information.hisaca said:Hello,
In my online receipt, there is a column for client ID, what are we to fill inside there.
Will my form be rejected if I leave it blank.
Please help
I have a refused TRV in 2013 and what I have on it is UCI number. If I write the UCI when they are asking for client ID - are they the same.prprpr said:HI if you have never applied for any CIC related document like study permit or work permit or any other permit, you will not have a client ID or unique client identification number (UCI). Write the same UCI which you wrote on your generic application form.hisaca said:I have a refused TRV in 2013 and what I have on it is UCI number. If I write the UCI when they are asking for client ID - are they the same.
If I write the UCI on this receipt, won't they think its a TRV I am paying for whereas this is PR. I am afraid of information mix up
