Got medical examination request on 17th Jan. The letter says that they have completed the initial assessment and expect to finalize the application without a personal interview.

No, I have sent only one application and have I never withdrawn my application.

By the way your case is little bit confused to me as, you have submitted your documents two times. But, why? would you please clear your case to us?

Initially when I aaaplied PR, I was in US. After that my company has been taken over by TCS. Due to that I came back to India. I have communicated the fact to Buffalo VO that I came back to India. So they have returned my application stating that I have to prove that I have been legally admited to US and stayed in US for at least 12 months continuously. So, I have sent the copy of my passport which will show the entry and exit stamps which will prove that I have stayed in US for more than a year. After sending my pasport copy they have accepted my file and given me the 2nd AOR. So, that is the reason I have sent my documents twice.

If you not applied for withdrawn of your application, then it seems to be an error...

btw, why your status is showing file transfer with two different dates?

On the first date, my file transfered to Buffalo and on the second date it has been transfered to Seattle. But for some reason it shows the same Seattle for both the dates.

I have received 29 pages CAPIS NOTEs, with nothing into it. just they kept open and updated my file that many times. However, they haven't started eligibility nothing else.

CIC is not clearly mentioning what their policy would be with the huge stockpile of applications.
applicants have been kept in darkness for quite a while now.
the standard reply was it would take 2 or more years to process.
for many 2 yrs have passed. nothings on the horizon yet.

Great!!! What actually is deserving is that Pre June applicants shall receive MR by 24 months from the date they sent full documents. In this sense, most of the pre june applicants have passed 15 months+ and should hear some good news by August/September 2012.
