Family Information Form (Pakistani Students)

Dear All,

I wonder wether to use Supplementry information or IM 5645 form (as both forms have almost same informations)
or both forms go with application.
Kindly advise

Just checked Pakistan.gc.ca website. It is supplementary information form. (old one). Anyways, main application form + supplementary form+checklist.

both forms won't go together with your application . Just send supplementary form.

actually both forms are available on CHC, Islamabad website at diierent locations.
the new family information from IM 5645 has been added recently.
There is 19 20 difference, If u can kindly put some efforts to check and compare both of them.
I think i should submit both of them....

Ok. Just compared both. They did not update it on pakistan isl website . The difference is huge. Now they are asking for present occupation of each family member and also want to know if they are deceased or not. I submitted old supplementary form again in order to keep my info constant with my previous application. Since you are a fresh applicant, you need to submit the updated one. The difference is huge if you can decode the underneath meaning of showing deceased parents and their present occupations. The updated one exploits more hidden info. In the old form, you could hide if your parents were dead or retired.

Now there is onething more if you could please guide me.

I have 4 sisters...No.1 and No.2 of them have got fine date of births and are married but there is a problem in date of births of no.3 and no.4
there is a difference of 5 to 6 months in their birth dates only in documents like no.1 is born on 1-jan-1990 and youngest one have got 1-jun-1990. so should i mention them with their dobs or i should hide these info.

Since they are not going to ask any documents about your sisters. You should write the correct date of birth of the youngest one for the sake of logical sense.

It was not a serious matter. You were allowed to make one typo. instead of writing 1-jun-1991 or 1992,you wrote 1990 instead. Aik button to ghulat dub Sakta hai

You should just relax they are not going to ask anything about your siblings, documents wise.

With either options, you are fine. You could not leave it blank, though

iPhone is always in my hands. Youtube,skype, the whole world is on it. You have no clue. Right now I'm under my bed sheet listening to
Music in my air conditioned room . In terms of flexibility and having access to Internet, iPhone beats your desktop pcs and laptops. My day to day work never gets disturbed because of this flexibility with iPhone. Trust me, I do more work in my house then you probably do.

Provide them both forms...Supplementary+IM5645

They know better wt to do and how to do?

This forum is informative but it also create confusions...i would prefer you guys not to be addicted and it all depends on luck.
If your dana pani is written in canada then nobody can stop you ...not every1 provide 100% correct documents, there is something missing in every1's file which might a document or a piece of information.
I know a guy who applied last year with 50% fake documents and he was granted a 2 years multiple visa, so as i told it totally depends on luck.
I see here some guys are curios just to get visas and they have no concerns with education. I believe if some1 want to get higher and quality education, he/she should get admission in university not in some college. It makes strong not only your visa file will also help afterward in job market. Most good employeers in almost every country of the world prefer university graduates than those who are graduated in colleges.
so before applying just think and realize and never be worry about that either you will be granted a visa or not.
Hope my points are not objectionable here...
