Document 2 is my Medicals. As soon as I entered Express Entry I found a CIC certified doctor in Toronto and went and did my Medicals. As per the date it was 06 October 2015.
Is this OK? My understanding is that they are valid for 1 yr and at the time of the E-APR have to be valid for 6 months. Is this understanding correct?

You can use that medicals but I strongly advise you NOT to use it.
It is best if the medicals is done as late as possible. This will give you a LOT of landing time. After you get your visa, you should land BEFORE your medical expires and medicals are valid only for 12 months. Assuming your PR process takes 6 months, you might have only very few days to land. Please do not make this mistake.
I strongly suggest you to get a fresh medicals done now and then submit the application.

It is unlikely that you will get a job from Canada while you have still not landed there (plus, your profession is regulated AND you need a license so I would say, the chances are practically zero).
2-How can I apply to different provinces for Nomination ...like Ontario or any other province in Canada,without being nominated by that specific province while being in the pool ? I am waiting for the new rules to be announced in Jan and Feb for different provinces..
Can y tell me which provinces don't require a job offer letter and sphere I can apply to that province directly without a nomination from them..

IELTS is the best and the quickest (and relatively the easiest way) to increase your CRS. Apart from that, you could get the ECA and IELTS for your spouse (if applicable). Otherwise, securing a PNP is a long (but effective) process that guarantees you an ITA.
