When you fill out the bls application form, you are suppose to mention the addresses you lived in India and the duration (As they collect clerance check from all local police station, you mentioned). I don't know if they will go back and refer these addresses while your BG check, if they do so, I'm sorry to say that might be a problem. IMO
 
I applied for India PCC from Indian Consulate at Houston, USA. The form had only one field for Indian address: Permanent Address in India. And the PCC itself doesn't have any Indian addresses.

Have you lived at more than one addresses in India?? if yes, have you declared the same in your personal history?
 
Do we get a "Your application(EXXXXXXXX) has been updated" email every time any of the statuses change?

I don't know, how it works but BLS forms have multiple fields to input your address, duration (From-To)

Sure, please go ahead and use it. You don't have to ask me to use a template.

From my personal experience, no. Sometimes it is triggered very late (2 - 3 days late) and sometimes it isn't triggered at all.

For Indian PCC, it's often better to just go with the address mentioned on the passport.

I did the same. But just worrying about the other addresses the i mentioned in my personal history.
