The easiest way I found was to use the Snipping Tool to capture the document, then print it to a Letter size piece of paper.

Thanks for that! Also, in Annex A, I need to fill up only details of primary applicant or all dependants as well?

i just printed out the whole email with the annex A

lol, so naive...

single applicant means - 1 x less PCC, 1 x less medical , 1 x less ielts+education to assess etc (thats assuming its only spouse, with kids you can add more) obviously their file will be a little more complicated. also it depends on whether they have any travel history or not... no travel history means slightly lesser number of checks also...

to say they are "priortising" single applicants only is a little absurd... :) based on your input then most people who have got PPR are indians, doesnt mean indian applications are being priortised ha ha... it just means there are a LOT of indian applications, and applicants on this forum

HI Krout! Thanks for the input. It was announced that as of December 15, 2016, New Application in January 2017 no longer need to submit medical exam sheets for sponsored\accompanying family members until it is requested by CIC and the medical form issued then. This is only for application for sponsored family members.

The panel Physician offices in my country misinterpreted the information and applied it generally to all application. Therefore, I had difficulty securing the appointments for my non-accompanying family members. This medical form that the office was requesting was for family members that were being sponsored. On the other hand, my application was Express Entry which does not apply whether I have family members accompanying or not.

I therefore uploaded a letter of explanation requesting assistance but to date there is no response. Fortunately, the situation was addressed and the medical exams were done. But now I have no where to upload the document.

I will upload using the Webform option (CSE) as you indicated. Anymore assistance will be greatly appreciated.
