Request for Passport mail in my inbox - woohooo!!!

FSW Inland applicant - AoR 27th Dec 2016.

Nothing changed in my application except medicals passed. not even background check changed - it was always in Not Applicable state!!!!

Could anyone enlighten me with regards to the next steps once PPR is received. I am based out of Delhi so should I straight walk into VFS office at Shivaji Stadium metro station near Connaught place with my passport, copy of CPCO Final Request e-mail and 2 photographs?

Queries:
1. Do I need to take an online appointment for visiting VFS in advance before visiting their office at Shivaji stadium?
2. The 2 photos should be glossy or matt finish?
3. How much fee do VFS charge for this service?
4. In how many days will I receive my passport back?
5. Is there anything else that I need to carry?

I request members who have already experienced, kindly answer my queries. It's little urgent.

He is Dec-16 AOR, FSW-O, w/ spouse, RPRF paid upfront
PPR email was received in the morning, most likely he got it from Ottawa.

mine was glossy. No issues faced
3. How much fee do VFS charge for this service?
approx 650 Assuming you opt for SMS service. For courier of passport another 300 or so
4. In how many days will I receive my passport back?
three to 15 days
5. Is there anything else that I need to carry?
photocopy of passport

Who knows how to fill the AnnexA?

I just printed the Annex A form from the email. In the email, it says that there is an attachment at the end - actually it is just an a table with your name and place to add address. I hand-wrote after printing and sent it with my passport and pictures. Thanks
