Inland and outland are made up terms - IRCC doesn't use these AFAIK. Applicants from the US are outland applicants IMO.

You call it a ghost e-mail only when there is an e-mail that says there is an update to your profile but you do NOT see any updates on your profile whatsoever.
Off late, some applicants have not been receiving any updates on their myCIC account (they login at a random time and see an update such as medicals being set to "Passed" or a change in BG status.
