I applied via mail in NYC a couple of months ago.

1. Only the new passport is required
2. I remember sending only the bio pages from new passport (first page and last page) + latest US Visa page.
3. My passport was renewed in NYC, so I took a money order for $27. My wife's passport was from India, so I took a money order for $42.
4. Self-attested

The processing was quick and I received the PCC within a week.
I just mentioned "Immigration" as the reason for PCC. So, I got a call from embassy asking which country am I applying for and confirmed that I will receive the PCC in two days.

Yes, one money order incl. processing fee and welfare fee.

On the reason for the request, I don't think there were any fields. In the miscellaneous application form, all you need to tick the police certificate. I added a simple cover letter (which is not mandatory) requesting PCC in which I stated "for immigration purposes". I'm not sure whether that triggered the call but my PCC says "no adverse information found which would render him ineligible for the grant of travel facilities including visa for CANADA".

@american_dream : While filling profession and business address - What should i fill? employer location or client location [both are different in my case]

Also did you include one or two returning envelops?

I filled in the client address where I work.
I sent both passports together in an envelope and sent 2 return envelopes. But they sent both passports in the same envelope, and it had the other unused envelope as well. :)

To add, I sent 2 envelopes as I thought we will have different processing times and they could send them separately. But both were processed together...

Hi, likewise my passport is issued by NY embassy and my wife’s from India. Were there any physical police verification checks at your wife’s address mentioned in the passport?
