Are you already a PR or a citizen and you are sponsoring him or your spouse is sponsoring you and him? If so, excessive demand on health care would not be a reason to refuse him.
If you are applying for your immigration as well as his, excessive demand would be a problem only if immigration feels that his care would be too expensive. I have heard that their limit is around $5000 a year.
