Medical Form is pre-filled one by Visa Office itself. You don't need to fill the form. It will have IME number and bar code

You will get that from from Visa office and same has to be taken to DMP for medical.

Hello Quorax/any seniors,

I have one question regarding the medical form to be filled for a child under the age of 16. Could anyone please help me who/ what kind of medical practitioner can approve that form? Is it the one who is allowed to do the medical or anyone from my country?

Can I put my address as in Dubai and Visa office of India? I have to yet got my proof of admissibility for entry of 1 year. So can I keep my address of Dubai and put visa office as Delhi?

How many months you have completed in Dubai ?. You have address of India where all documents will be reaching safely ?.

Please put email address and you will receive all correspondence by email (except PP and COPR at end). You can change the address at later stage. Not an issue

Just remember that you will be requiring Passport at Delhi and not London at final stage. So think of that also.

Your family doctor or any clinic should be able to do this - We had done it through Unicare medical in Bur Juman and the doctor had charged us AED 100 for 2 children

It took long long time for me now 9 months passed from med exam and untill now no PPR,

BTW last email LVO sent me background check will take several months till complete
Why they didnt mention how many months and they leave it open in several months???
