Doubts during filling form for FSW

Hi,
I request some experts to help me in completing my form for immigration application under FSW program. I am not using a representative, thats why I am having a few basic doubts.

Form- Permanant Resident Program.

1. Question No:1:- Family Name and Given Name, can some one clarify on this. In my case my name is John Mathew-- John is my name and Mathew is my fathers name. Actually I am not using Family name or Surname in my name. Later mentioned name is same in Passport too.. My family Chenatt and this is not shown as part of my name anywhere.

So what to write for Family Name:........ and Given Name:.........

2.Question No:12:- A simple doubt, do we consider our Lower Kinder Garten and Uper Kinder Garten (1 +1)= 2 years as part of education.
Or we should consider only Elementary= 10 years and Secondary = 2 years and Professional = 4 years, So total 16 years.
If we consider kinder years, it will become 18 years. Should I consider kinder years also?

3. Question No:14:- Mailing and Residential Address
Currently, I am residing in another state and as in due course, I may have to move from here due to professional reasons. So can I mention my native town/state address as mailing and residential, so in future I can avoid the need for changing of address. Please see that, I am not staying in my native state/town but I want to maintain that address, Can I do it?

4. Question No:20: Where do i intend to live in Canada?
If I mention a city after my preliminary studay and mention it here. Can I stay in some other city on getting PR Example, if I mention Ontario, Can I move to Toranto later. What is the relevance of this question.

5.Question No:21:- Family Members.
Should I mention my Parent-in-Laws too, as in future if I want to bring them to Canada. Or I can mention my spouse and my parents only in this boxes?

6.Question No:22:- If so will they accompany you to Canada.
In case of my parents, they may not accompany me to Canada immediately. They may come only after few years. So should I "Tick" on box for will they accompany me to Canada.

7. Question No:3:- Other name you have been using.
Can I mention my name which is in Church. Its something like other name.

8. Question No: 11:- Personal History till Date, what should I mention in "From - To" in case of current job. Should I leave the "To" date field BLANK or should I put date of the FORM COMPLETION DATE.

9. Question No:18: What is the relevance of address, which i lived from age of 18. Should I provide any police verification filled from all these places or is it only for information. As I am into consulting, I travel a lot and for short durations I stay in client locations for 6-8 months. If I mention all these Guest House/Hotel address, it will be a tedious job to get verifications done. Can you suggest an easy way for this. same time, I was having a house in my Base location of job. Can I put this home address for these durations together by omitting Guest House/ Hotel addresses.

Fee Payment
10. Question No:1: Calculating Fee.
Right now, I am planning to move only my spouse along with me.
So my fee will come
principal Applicant- 550 Can Dollars
Spouse-22 years above- 550 Can Dollars

I am not planning to take my parents immediately, but in future, I may plan to move them there. Should I have to pay the fee for them now?

1. What is your legal name? That should be what you put there.

2. I would not count kindergarten

3. If you have an address in your native town where you can receive mail, you can put that as your address.

4. Ontario is not a city. It is a province. Toronto is a city in the province of Ontario. It does not matter which city you put on that form as long as that city is not in Quebec. Toronto is a good city to put down.

5. You should only mention your parents in the form called "Additional family information". In your application form, you should only put yourself, your spouse and your kids.

6. Your parents should not be on that form anyway

7. If you have a secret name only for church, you can mention it there but it is not necessary.

8. Write "present" in the "to" field.

9. You do not need a police verification for each address but you should put down every address you've stayed at as far as you can remember, especially addresses where you've stayed for some length of time like 6 or more months. When you are asked to provide your additional documents, you will have to provide police clearances for every country where you've stayed more than 6 months.

10. Your fees are correct but there is another fee if you are granted PR. You can pay it also at the same time to save time or you can pay it later when you are asked. That fee is 490 per person. You can not apply for your parents now. You can sponsor them after you get your PR but it takes a long time, at least 3-4 years to get them to Canada.

Leon, I need a little more clarification on question No 9.
During last 2 years, I was within India (never changed country) and was staying in different states as part of my job for nearly 8 months each. My stay was either in a company guest house or in a Hotel. During the same time, I was having a house rented in my base location of my office in another state.

First of all, can you elaborate on the police verification process. Do I need to get a police verification done from the places which I stayed for more than 8 months. Why I am a little concerned is, if I go to Police in those location and ask for verification, I may find it difficult to get that.

Or can I just mention my entire stay under my base location house.

Please brief me on future requirement of police verification linking to duration of stay in a location.

Like I said, you do not need a police clearance or verification for each address. You only need a clearance for each country.

Leon, could you tell me the difference between a "police verification" and a "police clearance"? I ask this because I stayed for some time in Canada and I am worried about starting a long process while being in my home country. Thanks in advance.

You have to ask the original poster what he means by verification. As far as I know, immigration only asks for clearances. They want one from every country you've lived in more than 6 months since the age of 18. In most cases, you can order them through the mail.

Ok. Just in case I have to order mine how do I do it? is there any resource online or should I contact my landlord from Canada.

You get a police clearance from the police, not from your landlord. Contact the police in the country you need a police clearance from and ask them what you have to do.

Like I previously mentioned, I have left Canada and I'm back in my home country. My touch point would be the landlord first because I have his contact numbers and not the local police person from that city. Thanks for your advice though, Leon.

Strangely, my first idea to look for contact info for the police would be to google it but if your landlord can look it up for you, that will work too :D
