Documents to send with CAQ

I have completed my CAQ application online and made the payment through credit card. I am sending the declaration form, passport copy, Photo and LOA with the application.

Do I need to fill and send the following documents too? : http://www.immigration-quebec.gouv.qc.ca/en/forms/search-title/credit-card.html

I did not send it with my application, just add the summary of the online application and the checklist form..............
