Unfortunately dialysis will make him inadmissible because he will place excessive demand on the Canadian healthcare system. The fact that he needs a kidney transplant adds to the potential cost. Are your family members aware that they are unlikely to be able to get a license to practice medicine in Canada? Only a very small percentage of IMGs are able to get a spot to do their residency again and it is usually limited to family medicine. There are also a small number of spots in a few provinces where they train IMGs to be GPs in communities without adequate medical coverage. You still have to spend lots of time and money to pass all your tests before you get a chance at a spot. Think 1 % of IMGs might receive a spot maybe less. If they would like to practice medicine Canada is probably not a good option. Given your brother's kidney disease he likely would be medically inadmissible in the first place.

You can hire a lawyer but while your brother is on dialysis and needs a kidney transplant anticipate he will be refused due to the very high cost of his care. A lawyer can only prove that you won't be a excessive demand on the medical system. In this case I'm to sure how it would be possible.

Hi

Thanks alot
Then can we apply for PR or not

You could probably apply once your brother is back in good health after the transplant, however soon that is. Lawyers are good in dealing cases where there are possibilities proving presence of alternate cheaper generic medicines or something on those lines, but not for something straightforward expensive cases like this.

The health cost threshold is 19812 dollars a year so if the OP uses Google they will see what the projected costs of dialysis are per year , 60000 dollars.

https://www.canada.ca/en/immigration-refugees-citizenship/corporate/publications-manuals/excessive-demand.html

Here is an article for example laying out the approx costs as well as transplant costs and post transplant.

https://www.theglobeandmail.com/life/health-and-fitness/transplant-waiting-lists-and-dialysis-costs-grow-as-kidney-supply-lags-behind/article569465/

So immigration lawyer or not seems very unlikely that any application would succeed at least maybe until post transplant where anti rejection drugs reduce the annual costs. Even then not any easy path to approval assuming manage to get a transplant in home country which if anything like many countries could be a long wait for a suitable donor.
