Do i need medical again

My question is that, i took upfront medical on 23 October 2014 from India to apply for my open work permit and got it, and now i am in canada.
I got invitation and Now i am updating files for PR through express entry, should i take medical again? or how long my medical is valid, is it valid in canada also.

Incorrect. The OP stated the date was October 23, 2014. That puts the medical exam as over 2 years old. A new medical exam is needed.

My wife did medicals last month. March 2017 and then got sowp. I got the invitation this month and want to know if her medical Is still valid?
Jobit was in the same situation 2 years back and I want to know how he proceeded.

your wife's medical is still valid. you can use that for your application.
