Do I have to fill out the IMM 5540 Sponsor Questionnaire?

IMM 5540 was not included in my package nor is it listed on the documents checklist. Does that mean I don't have to do it? or am I missing this form?

I'm asking because I've seen different forms with different dates at the buttom. Is the IMM 5540 not neccessary anymore?

Oh yes..you must fill out the sponsor Questionnaire with out a doubt! Ignore the dates on the bottom of the forms..thats just when the forms were last modified!!

IMM 5540 is #7 on the Documents Checklist (IMM5491) and is a required form.

I don't see number 7 nor do I see numbers....but this is the checklist I've recieved with the package and I don't see IMM 5540. This is really confusing how they have many versions of this.... ???

This is the one I'm using "IMM 5443 (03-2008) E" version of the checklist

I just went right to the forms page and looked and its there so i dont know if you got it from another site?Just go back to the immigration canada webpage got to on the left side sponsor a family member then proceed untill it says how to apply then the forms will come up the complete ones. here is a link to the form http://www.cic.gc.ca/english/pdf/kits/forms/IMM5540E.PDF

What I want to know though is why did CIC send you that? Or any other different forms? There must be a reason for it... perhaps you should try to call CIC? I'm not sure why they would have that form or what it's purpose is, but if you got it directly from them you may need to double check. I do agree it's not what everyone else is using (from what I can tell) but it does leave some questions unanswered.

They just sent him the older forms???03 2008 the new ones are 04 2008 just a mistake on their part but just use the ones i sent to you and they are the newer ones.

Yes I agree 110% I mean I'm using something I've recieved from CIC...I think they only gave me one checklist and not the other one with numbers and all......besides the IMM 5540 was NOT included in my package, so I thought it doesnt need to be completed, but I think it's best for me to do everything on their site thats needed....better to be safe than sorry, no?
