Now it's clear... So, keeping in mind that this is not my job, I'm not a professional, and that you're free to do whatever you want, I suggest:
1) Get a document from the government (could be a copy of the law that changed the passport naming system) that shows why you have one name in the university doc. and another in the passport;
or
2) Get the university to issue a document stating that you are who you say you are;
or
3) Get someone in the government (there must be an authority responsible for keeping a registry of everyone in the country) to attest that you are who you say you are;
4) Go ahead with what you have and hope that the office processing your application is aware of the change in the passport system.
