CIC didnt specify which form I am supposed to use. I have found online IMM5562E that looks to be the one to use. Anyone has experience with that? Thanks.

Didn't you give that information when you were applying initially? Are you required to submit photos of your passport pages as well? Also could you please share what is your Background Check status?
I gave that information before - 10 years travel history, thats why I am suprised they are asking again. I have also attached all passport photos and my working permit before. My background is medical passed and avaiting for additional documentation.
Please I have a question and it is open to all to pls help me. In my statutory questions I ticked that I haven't been refused a visa before however I just realised that I was refused us b visa in 2015. I didn't remember at the time. Pls what should I do? And does anyone know anyone who was in this situation? Pls I really appreciate any advice from you guys thanks so much
