Please help... I am about to mail my documents for Spousal sponsorship but I just want to confirm at to,

what document checklist should we attach for my sponsor, IMM 5491 or IMM 5443?

He's in Canada and I am in the Philippines.

Are you using the sponsors DOB and surname and place of birth?
