Confused about IMM 1017 and eMedical

The checklist says we need to attach an upfront medical form (IMM 1017), so my wife went to a panel physician and told them she's doing a medical for immigration to Canada, and this is the form they gave back: https://i.imgur.com/0MXq4bn.jpg

Note that this form does not say IMM 1017 on it anywhere, but it does have the UMI at the top right corner. What do I do? I've already submitted my app and only realized this now.

The form that the doctor gave you is the correct one, so don't worry about that. IMM1017 is not used by Panel Physicians who use the eMedical system.

Ah ok. That's such a relief!

I did check off the box for IMM1017 on the checklist since I thought the eMedical form I got was in fact IMM1017. I just got worried for a bit because I didn't want them going through the app looking specifically for IMM1017 and then sending the whole thing back to me after not being able to find the form.
