You may call CIC call center and provide them your file number. They will give you start and end date of any study or work permit you held in canada.

Option 4 for citizenship. If the line is too busy you will get disconnected. If you always want to reach someone the best option is sponsorship

I mean we are husband & wife both eligible for application now. Should we mail them in separate envelopes or in same? Does it matter?
Also correct me if I am wrong, but there is no Family application forms.

The instruction guide suggests to send all family applications in one envelope. Yes, I don't think there are specific family application forms.
 
For Minor's application question 8 (Parents Details) only one parent name can be filled either father or mother, is that how you guys are filling the forms or please advise otherwise?

No, there's space for both parents info
