Could any one please advise me how I can get a copy of my last work permit that immigration officer took it from me while I did flag polling at border?
Unfortunately, I don't have its copy. Now I got stuck while applying for citizenship as I don't have the info of its issue date and expiry date.

You may call CIC call center and provide them your file number. They will give you start and end date of any study or work permit you held in canada.

Option 4 for citizenship. If the line is too busy you will get disconnected. If you always want to reach someone the best option is sponsorship

What is a good way to apply as a family?

I mean we are husband & wife both eligible for application now. Should we mail them in separate envelopes or in same? Does it matter?
Also correct me if I am wrong, but there is no Family application forms.

The instruction guide suggests to send all family applications in one envelope. Yes, I don't think there are specific family application forms.
