13. Have you ever had immigration or citizenship status in any country other than Canada, including your country of birth ?

If your country of birth is not Canada, that means we need to mention the country of birth in the table right ?

Include your country of Birth as well. Choose citizen.

Thanks for the advice. Didn't know that we are attending an English class @ 2 AM o'clock.
FYI - The actual English way of saying it is: "Fill in the form" ... Just a tiny detail that might help us all in the ESL , right? ;)

Hello All, Does anybody know what is meant in Q 11. Its the one with employment history, physical location, name of contact etc ? What physical location, like home address or job address. And same with " name of contact" ? Is it like a reference at your job?
 
I have the same question too..
I put my home address, registrar's office in Name of contact, and I put school phone number and email.

I can't even print it out though. Who the heck made this stupid pdf file.

In case of being unemployed, what should be mentioned for name of contact, phone and email ??!!

I'd suggest put your name and personal phone number.
The problem with this table, it is like they forgot about it and just dropped it the last minute. so many detailed are requested, yet the table is so small; all will fit in one row? I remember a similar section in the PR application and was well defined.

If you put in more info and move on to the next field. It automatically resizes itself to show all the data entered.

yes, answer is citizen, from date of birth to current, if you still holds it.
