Even with the FIFO method, not all timelines are the same ... some are straightforward for the VO, and they get processed right away, others take longer to process, so even though they may get picked up by a VO for initial assessment using the FIFO method, they don't always finish within the same timeframe. So a VO might pick up 3 cases, one which he gets completed in say 15months, another in 18months, and then another in 30months, but since they are also picking up a new file after finishing the first application, then the 4th application in line (assuming no one else picks one up in between) will be terminated at the same time as the third (3rd one took from month 0 to month 30, but the 4th one took from month 16, ie after the first one finished, to month 30). its this different processing times that mix up the true FIFO methodology.
it would be even worst if they did it by batch and had to hold back a batch just because they were still processing one application of the 10 (for example) that are in that batch. when you look at the GCMS notes, the VO will include a BFD (bring forward date) which is a reminder to them on when to look at the file again if no new documents have been received yet. It usually happens that they will request document from you, or for a security check, medical, etc., and they place a BFD depending on when they expect the document back, so the longer these documents take to return to them the longer your processing becomes.
also don't forget that the medical report comes from new delhi, they are the regional medical office and your medical exam results go to them for review, so that adds delays because they process the medical exam results of multiple CHCs in that area of the world.
So even though the file is picked up in a FIFO fashion, all of these factors play a role in changing the processing times so that the files are not finished in a FIFO fashion.
