Please help as i am confused which form is needed, below is the detail of the letter. Which form they are asking to send

The letter that i received says to comply with all requests marked with an X.

X Please properly and truthfully complete the IMM8, Schedule A and IMM5406 and supplementary information/afghanistan questionnire...........

Are they asking to fill all the forms or just the form with the X marked in the link? I filled all the other forms such as IMM5406 and IMM5669 when I sent them my application forms at the begining.

Also what is supplementary information/afghanistan questionnaire forms. can anyone please send the link.

Doesn't matter if you sent the forms in the begenning, they want updated new forms, you have to VALIDATE them on your computer before printing, it will give you barcodes too.

For the supplementary form (the afghanistan is marked I see).

Dude, I got the same request! LOL

I sent them the forms originally as well, but as lonelyinca mentioned...we need to resend updated forms. So make sure you got all the new updated info on there. I also have to send additional proof (pictures, proof of visits etc.). I think after this its 2-3 months wait for them to process and then request remed + ppr...can Khilafah please confirm? thank you bro!!!!

i got the same requirements. they are asking u to fill main form imm8, imm5406, imm5669 and supplementary information form which are available from the link checked by them.
actually everyone who applied during or before july 2011 is required to fill new forms

ya, I see the same problem, may be its new issued number and their system not updated yet.

there is something wrong with the system,i have tried with both F000000000 & B0000000 but no result whereas i have send them required documents

How do you send mail from afghanistan to islamabad. i have like 10 days left to send everything and i am not sure how can i send mail to islamabad from kabul in an urgent manner. please advise.

maybe you are entering incorrect information, or they made a mistake in your date of birth or something and so the credentials aren't working for ya...

You may have to enter the sponsor's/applicant's info depending on who's file# you're using

Yes, as I understand (instructions from here and CIC website, just recently ordered).


Need the Principal Applicant from Pakistan to fill in IMM 5475 which gives CIC the consent to release his/her info. to you, the Sponsor.
Fill out IMM 5563 yourself, has to be the Canadian Citizen or PR: Access to Information Act, fastest would be email and Electronic Files for Status of Application...don't do Physical Files or it will go to Islamabad and slow down your case.

Lastly use their online payment site to pay $5.00 and attach that receipt to the email. Also since its your first time, you need to attach proof of Citizenship or PR (passport, citizenship card, PR card etc.).

Send to the email address on bottom of IMM 5563.
