what's your timeline? You can go into the payment webpage and from where you can see whether you need to pay for your son or not. They have the guideline

Schedule A: please provide a separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature with no gaps in time started from march 23, 2008 regarding question 12.
> Client Information: please provide a pay stubs from your employer.
> T4: please provide a copy of the Canadian Revenue Agency T4 form for year 2014.

> Notice of Assessment: please provide a copy of the Canadian Revenue Agency Notice of tax Assessment for year 2014.



do we need to send everything via mail or email. It asks for my original signature for schedule A?

Also, i changed my job in jan 2015. Should i add my offer letter for my new job as well?

They don't specifically say to email the documents, in that case, I would send them via registered mail in an envelope to their office. You do not need to include the documents from 2015, but if I were you, I would do so. Just be open and honest about everything and don't let any questions arise. There is absolutely no problem with changing your job.

You always include a letter out of pure courtesy and as response to a request. ;)

Congratulations, I have the exact timeline as you. Except I haven't received any doc requests. Hopefully I can get medical request soon.

October 20th applicant. Got an email from CPC tonight requesting the following:

Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Please provide a new updated Schedule A with information for the last 10 years until present day. This must be received at this office by: 2016/04/22

i suggest if you dont hear from them after giving medical till march 30th. get the rcmp criminal record check upfront with you as it expires in a year and they will ask for it again which will waste another 3-4 weeks..
 
I was asked for T4, NOA & copy of new passport, any idea how much will it take to get medical request? I am too stressed.
