That's really bizarre - what's a lawyer going to write, "yep, that's a name change document all right, appears to be valid (but I'm not the government, I'm just a lawyer)."

It also makes no sense - in Ontario anyway, all lawyers are commissioners to take oaths - so it's practically nonsense, an affidavit is a statement by someone made in front of a commissioner, and here they're asking you to get an oath from a lawyer, who will, I guess, affix his signature saying "I said this."

Anyway, I'm not claiming you've got it wrong, and you can't argue with them if that's what they want, I guess. But as a requirement it doesn't make much sense.

Here's the general form for an affidavit, but again, I think the requirement's silly, this is just to show the facts:

Bad part is that its a $60-$80 to get an Affidavit here in Winnipeg. I truly don't understand why CGI cannot accept just the name change certificate from the province as during the name change process here in Manitoba you will need to provide your Fingerprints and also need to notarize a Vital Stats form by a lawyer.

I don't understand either. On top of that, all name changes in Ontario anyway (and I think all other provinces) have to be published in the official Gazette, available on the internet. Geeky stuff but the name change order could be checked in five minutes on the internet. (I suppose the lawyer could put that in the affidavit).

Anyway, count blessings that they're not requiring the doc be apostilled or whatever, that would be way slower and probably more expensive.

@GokulGuruprasad @mumbaikar I got a response back from CGI on OCI with the name change, they said they would need an Affidavit apart from the Name change certificate from the province.

Let me know if you need any other information.
I got good news from Service Ontario today. They said that my name change certificate was printed early in the week and should be in the mail now.
I hope to receive it by next week.

Post that I have to change the name on all my official documents such as CRA, SIN, Driver's License, Health Card, Car Permit etc.
For now I'm taking it a step at a time as I haven't even applied for my Canadian PP yet.

@navid2014 I hope you didn't get any trouble with getting a Canadian passport with the name change certificate only? Hope they didn't give you any pain to force you to get a new Citizenship Certificate with the changed name?

To be frank, I was expecting trouble at the Indian passport surrender & OCI stages ;)

You absolutely do NOT need to change the name on citiznship certificate - your name change document + the old citizenship certificate will be enough for those purposes (one to identify that you-are-citizen and the other that you have changed name).

(Note you may need to provide some other ID like driver's license etc as well - check the requirements. I'm just addressing that you do not need the citizenshp certificate. Somewhere I have a link to precise government instructions on that bit but too lazy to search right now).

Yes you are correct you dont need to worry about updating Citizenship certificate with new name, as I got the Canadian PP with the new name with old Citizenship Cert & previous name on it.

I would recommend you apply for Canadian PP right away without further delay.

Man.. these guys are "hardcore".... forcing us to put XXX in case the first name/last name is missing ;) ;) ;)
I gone through that alot and then changed my Name (In two parts), Applied for new certificate , and then applied again passport , total process took alot of time

good luck sir :)

I am surprised reading this thread. My father in law got Canadian Citizenship
where

OLD INDIAN PASSPORT
FAMILY NAME :
GIVEN NAME : Real Name

NEW CANADA PASSPORT
FAMILY NAME : Real Name
GIVEN NAME : XXX


INDIAN OCI CARD
FAMILY NAME : Real Name
GIVEN NAME : XXX


While applying OCI, BLS processed it and no name change certificate, or any other documents was asked.

I also got good news from Service Ontario today, they approved my namechange application and the certificate is on its way.

I know first thing we need to update is the Driver's License and Health Card. Did you apply for passport yet? As we need one ON ID for passport application, Do you need to wait for the new Driver's License card to arrive or the passport office will accept the temporary paper one with the old card?

What about other places such as CRA and Service Canada do they also need an ID in the updated name or they will just accept the name change certificate and citizenship certificate?

I have seen cases where people have applied for surrender certificate before (or in parallel) applying for Canadian passport to save some time.

Yes I applied & received Surrender certificate. Canadian PP is not mandatory for Surrendering Indian PP, you just need to submit a declaration in absence of your Canadian PP.

While filing the Surrender application just go with the name as in your Indian PP.

I updated my Driver's License and Health Card.

I'm waiting for the proper driver's license to turn up in the mail before I apply for a passport - I don't wanna take any risks with the printout.
