Someone else on the forum recently had a similar issue with the 'biodata' form -- it did not get attached to the email even though it was referenced as an attachment. My understanding is that they contact Ottawa via email and received a response that included the attachment. Hopefully you will also see a response with the required attachments soon.

just to share to everyone.. i got my passport request today.... around 6:07 pm. i filed my pr application last may 2011... it was a very long-waiting period... hopefully, everyone gets the same good news too!

Same here i received a similar email, like you i'm confuse about the police certificate. Any body in the forum can help us?

Just found out that my application might be put on hold until my wife can take her chest x-ray. She's pregnant for about 22 weeks which means that she can't take her chest x-ray. The due date is IRONICALLY July 1st. What a day to have a birth. We've been waiting for PR for almost 2 years now. She can take her chest x-ray after July 1st. If they put it on hold, we probably need to wait another year or more. WTF!! I'm so pissed off. Is there any way that we can extend our medical results that we did in Oct 2011? Does anyone have a similar experience? I was expecting my application can be done in a year or less because I'm a provincial nominee. I've been in Canada for ten f*** years! Man I need a drink. >:( Oh, don't get me wrong. I'm super duper happy with our baby. :)

I am confused, could anybody help me. When the CPP-Ottawa ask for the personal history updates then what should I do. Fill the form IMM 5669E or just make a table as a word document and send it. Please help!!!!

I made the same table as in IMM5669 (personal history) in word format and emailed it.
