And further it says on first page that consent to be signed by all persons over age of 18 for whom information is contained in file.Though i am principal applicant but if i have to get it signed from my spouse as well as she is on the application as well.If she has to signed the separate form ?? And if 5 dollar fee is to be sent by ordinary cheque.

> do you have any relatives in Canada? you can ask them to do it for you, just sign the consent letter. I don't think they are accepting ordinary cheque but you can always double check it from the link that I gave you. Read it carefully and it will guide you accordingly
And last where to send the form.
