I have the examples contradicting the claim.
My first cousin NOC 4131 in Medical college got the visa in Jan 11 from Islamabad office.
Her friend a pharmacist (Still a high demand occupation in Canada) has still not received medical.
They applied together in May 2009.
There are so many reasons beyond your control that can delay your case.
FOR EXAMPLE
It depends upon your background. If you have lived a too many places the background checks may take more time. RCMP is responsible for background immigration checks in Pakistan and they have a staff of only 2 employees for this job.

BE SURE THAT
1- VISA OFFICE WILL VERIFY EVERY PAPER YOU HAVE SENT THEM TO ITS SOURCE BEFORE THE MEDICAL
2- THEY WILL CONDUCT SECURITY AND BACKGROUND CHECKS FROM EVERY ADDRESS YOU HAVE LIVED IN AFTER THE AGE OF 18.
More addresses means more checks and more delays which are beyond our control. So I think NOC has nothing to do with the delays.

In some visa offices they do not follow any order. Most of them process according to the NOCs. I posted my application in June and my friend posted hers in august, but because she is a social work her application has been processed before mine. I am still on the queue because i am 4131.
So it depends on the visa office.
