Birth certificate missing last name

Experts..pls help...

My birth certificate is missing last name..as sponsor for a pgp, I have submitted an affidavit explaining some other errors in birth certificate but has not pointed out the missing last name in birth certificate..should I submit another affidavit or fine to leave as is?
