Birth Certificate for FSW

Dear Seniors,
I dont have birth certificate, can I use 10th Marksheet and Passing certificate by CBSE as date of birth proof for FSW ?
CBSE passing certificate has just the father's name.

If yes do I have to give any affidavit for the same ?

You can use the marksheet as long as it has your birthdate and your parents' names. If not, then you can use an affidavit signed by your parents.

I dont have a secodary school leaving certificate. I just have marksheet which only has my name and the second document that i have is a secondary school passing certificate which says the following:

ALL INDIA SECONDARY SCHOOL EXAMINATION 1997

THIS IS TO CERTIFY THAT (MY NAME) ROLL NUMBER (XXXX) SON OF (ZZZZ) BORN ON (MM/DD/YY) HAS PASSED THE SECONDARY SCHOOL EXAMINATION OF THE BOARD HELD IN MARCH 1997 FROM XXXX SCHOOL

IN THE FOLLOWING SUBJECTS X Y Z

Will this be good enough? Do I need to send any affidavit as well? or a covering letter?

It should have your Mother and Father's name both. If not, then sending affidavit is better option along with cover letter.

You mean I should get an affidavit made for my date of birth and place of birth?? This affidavit has to e made by me or my parents? Can you please provide me a format for the affidavit?

Can I include the affidavit, secondary school passing certificate which has my d.o.b and fathers name?? and cover letter. would be sufficient but if possible get a Non Availability of Birth Certificate (NAC) from your local municipal authority.

Not true, there are acceptable alternatives as mentioned in the Guide and Document Checklist. Read and follow them exactly.
