Birth certificate alternative

Guys i dont have birth certificate.
I was born in village and my parents never bothered to get any certificate at that time.

Now its impossible to get any such certificate issued.
In my school records and passport my date of birth is mentioned.

can i submit an affidavit for the same? (maybe issued by my parents)
If yes what should be the format?

First thing you need to keep in mind is to submit a written explanation stating that why your birth certificate is not available. Then you have to mention clearly that what alternative document you are going to submit.

You can submit 10th grade certificate as an alternative provided that you full name, date of birth and at least, one of the parent name is printed on it. Else, you can submit an affidavit too as an altenrative document.

In my 10th grade certificate parents name is not there.
Is that mandatory? date of birth is there.

In any case i am attaching affidavit issued from my parents and other ID proof's for the same.
Hope they accept them.
Dont want my application rejected due to this reason.

I also do not have birth certificate with me. I am going to submit affidavit in place of it and my school leaving certificate. I hope it will work.
