This form is be filled out by you. Make sure to order electronic files only( and status of an application box) If you check off physical file then it affects processing time because they pull your file "off the shelf". and check off that you are ordering through access to info act. I ordered my notes via email both times and that worked well. The requesters info at the bottom is all your info (not your husband's). You have to also send along a photocopy of your permanent resident card to prove u are permanent resident. this page has instructions with it... Including how to pay the 5 dollars.

This is the form your husband must sign/date. You can fill it out if he can just sign and get it to u. But make sure it's filled out from your husband's perspective. Your info would be in the designated individual spot (step 4,5).

So when you order them you should have the following in the package
- form 5563
- form 5475
- proof of PR/citizenship
- check/money order or receipt for online payment ($5)

They used to have the address on the paper, they don't anymore, but this is where to send it....
