MEDICAL EXAMINATION:
 Within thirty (30) days of the date of this letter, a medical examination must be completed by:

When you go for your appointment with the Panel Physician, you must bring proof of your identity (government-issued ID card or passport).

After completing your medical examination and all required lab tests, send all the documents from the list below to this office in one envelope.

REQUIRED DOCUMENTS:

 Original Good Conduct Certificate (or Certificate of No Criminal Record) from the police authorities of all countries where you, your spouse/common-law partner, and each of your dependent children who is 18 years of age and older, have resided for a period of six months or more since turning 18. BANGLADESH POLICE CERTIFICATES. There may be more than one type of police certificate available in each country. To find out which one you need and how to apply for it, visit the CIC Web site: http://www.cic.gc.ca/english/information/security/police-cert/index.asp

If you do not provide the correct type of police certificate, the processing of your application will be delayed. Police certificates are valid for one year from the date of issue. If you have already submitted a police certificate which is still valid, you do not need to submit a new one.

 IMM 5669 Schedule A Background / Declaration form, completed with up-to-date information by you, your spouse/common-law partner, and each of your dependent children who is 18 years of age or older. Fill the form on the CIC Web site then print it: http://www.cic.gc.ca/english/pdf/kits/forms/IMM5669E.pdf

You must provide complete details of your personal history and addresses since the age of 18 (FROM 1982 TO PRESENT). Your spouse must provide details of her personal history and addresses from 1989 to Present. Do not leave any gaps. Failure to account for all time periods will delay the processing of your application.

 Pay the Right of Permanent Residence Fee (RPRF) for you (and your spouse/commonlawpartner, if applicable). The instructions are attached.

 Four (4) passport-sized photos, taken within the past 3 months, of yourself and each of your accompanying family members. On the back of only one photo of each person, write the person’s name and date of birth. Please do not staple the photos. The photos must meet the specifications available on the CIC Web site:

INSTRUCTIONS FOR SUBMITTING YOUR PASSPORT(S):
Submit your passports together with the documents on the check list above. Exception: If the Panel Physician advises you that further medical tests are required, do not submit your passport(s) now. Instead, proceed to submit the other required documents, and your passport(s) will be requested after satisfactory medical results are available.

 Passports are required for:
1. You
2. Your spouse/common-law partner (if applicable)
3. All dependent children who will accompany you to Canada (if applicable)

Before you submit your passports
 Check that there are enough blank pages in the passports to affix permanent resident visas for you and your accompanying family members.
 Check that you have ordinary passports, because permanent resident visas for Canada cannot be issued into DIPLOMATIC / OFFICIAL passports.
 Check the expiry date of the passports for you and your accompanying family members, because a visa cannot be issued with a validity longer than the passport.
 Fill out the form we have included with this letter (Appendix ‘A’) and enclose it with your passports.
 If you or any of your accompanying family members currently holds a valid temporary resident visa, study permit, or work permit for Canada in an expired passport, please submit with current passport.

IMPORTANT NOTES about Reporting Changes to your Personal Circumstances

Canada’s immigration law requires that you continue to meet the requirements of the Immigration and Refugee Protection Act throughout the immigration process up to and including the date you arrive in Canada. This
means you are required to inform this office, in writing, of any of the following circumstances:
✔ A change in your marital status (marriage, separation, divorce)
✔ A change in your family size (birth or death of a child or spouse)
✔ A criminal charge or conviction for you or any member of your family
✔ Death of your sponsor (Family Class applications)

IMPORTANT NOTES about Visa and Passport validity:

You should ensure that everyone in your family has a passport valid for a minimum of 12 months from the date of the most recent full medical examination. Visas can only be issued to a maximum validity of 12 months
from the date of your most recent Immigration Medical Examination (IME) assessment. If your family has medical examinations on different dates, the validity of your visas will be 12 months from the date of the first assessment.
If any one of your passports will expire before 12 months from the date of the medical examination, your visas will expire on the date of the first passport to expire. If your passports will expire soon, it is in your interest to obtain new or updated passports to ensure your visas are issued with maximum validity.

 To submit your passports, follow the instructions below for your country of residence.

You must comply with the above requirements within thirty days (30) from the date of this letter.
Failure to comply with these instructions may result in the refusal of your application.

After you submit your passports

 Please allow one month for permanent resident visa(s) to be printed and affixed in your passport(s). Do not contact our office to enquire about the status of your application until after one month has passed.

I am surprised to see that they have not asked for any Bank Statement/Net worth statement...
Why?
 
Back after more than a year, wanted to share some updates with friends here. I landed in Toronto on December 2013. Stayed for two weeks then went back to USA and now I am in BD. Will move in Calgary in March 2015.

Whoever I asked most of them discouraged me to go to Toronto saying that competition is fierce there and availability of job is also limited. The government has several agencies operating free of cost to provide initial support to new comers. Job searching workshop, how to write CV and, if needed they also help to find accommodation.

Want to build a career in social/community work in Alberta hopefully.
