What we did was wrote the explanation as to why I had not applied with my family, and explained that my wife maintains her birth surname and my 2 minor children use my (father) surname (this is evident from the PR card, landing doc and passport copies my wife sent with their application). We also included copies of my passport, PR card and landing document which shows my surname which is same as my 2 minor children. The letter was addressed to the Manager Programs Support Unit (as indicted in the letter you have received). The address is on the top of that letter, though mind you it is a P. O. box address, therefore I used purolator as I wanted a signature to confirm the delivery of my letter so the address is the same as where we initially sent the application -

I think my citizenship application is complete in all other regards. I got the help of settlement worker to vet it a number of times. I remained for more than three years in Canada. Underwent an English language training for more than two years.
However, I must also mention that my husband, who is the main recipient of our permanent residence, is yet to complete his three years. So he will apply later. I have provided birth certificates for my children, showing me as the mother and my husband as father. However, when he originally applied of permanent residence, at that time the birth certificates were only in his name. The children passports also carry only their father' names. The PR cards, as the children were and are minor, also carry his signature.
We got new birth certificates with inclusion of my name as the mother, and submitted these with my application to prove the parentage. So I sense that the parentage may be an issue for my case being referred to PSU.
I will also contact the CIC number and as you advised also prepare the letter with all the explanation and send it to the address.
