I cannot exactly remember how much my hubby paid for his medical exams but it's around 3000-4500 per person.
Step 1: Gather your documents.
The forms, both for the sponsor and the sponsored person, can be downloaded at the CIC website (cic.gc.ca). Look under Family Sponsorship. Download the checklist first. It lists everything that needs to be signed and compiled. Then download the guide. There is a guide for the sponsor (guide to sponsorship) and a guide for the sponsored person (guide to immigrating). There is also another form INFORMATION AND GUIDELINES which is country-specific. Download the one for the Philippines.
The guides are pretty straightforward. There is a step by step procedure on how you should fill out your forms. Also, it lists all the documents that you need to secure.
In the Information and Guidelines form, there is APPENDIX C, that you need to bring when you go for your medical exams. One copy for each person being sponsored. You also need to bring 3 copies of your passports, 8 pictures (the size of the picture is also included in the information and guidelines form, It is the Appendix B, I believe). After your medicals exams, you will be given a receipt, this is IMM 1017, that you need to include in your application (it is also listed on the checklist).
I suggest you follow the checklist. Everything is there.
Step 2: Send your application to your sponsor.
You need to send your application to your sponsor and your sponsor will send it to the Case Processing Center in Mississauga, together with the forms he filled out. Note: Your sponsor also needs to secure several documents like Confirmation of Permanent Residency or Citizenship certificate, Option C (general statement of his financial status from the previous taxation year, in your case that'll be 2011, if he already filed his taxes), if he can't produce an Option C, he needs to provide employment letter and pay stubs (proof of income, bank statements).
Your sponsor should also pay all the required fees to avoid delay in processing, you can calculate your fees at the CIC website as well.
Reminder: Before you mail your application to your sponsor, check your forms and documents a "hundred" times (of course I am exaggerating) to ensure that everything was completely filled out and signed and all necessary documents are complete. The CPC-M will return incomplete applications, or even those that are not completely filled out (like missing signatures) and you might want to avoid that.
Bottomline: Everything is on the guide and checklist, so read it carefully.
