Doing it upfront has nothing to do with it. If everything is alright during the medical exam, then they have no reason to ask for a second one unless some new information came into light! Call IRCC! This might have been mistake on their end as well! But for clarity it's better to connect with an agent and ask!
I digged a little bit more on how and when to do the medical exam, here is what I found:
As of October 1, 2023, you no longer need to get an upfront medical examination before you submit your application for permanent residence through Express Entry.
After you submit your complete application for permanent residence, we’ll send you instructions on how to get your medical exam done. Please wait for our instructions before going for a medical exam.
It make sens now.
I'll keep you guys updated, hopefully I am close to the goal.

Same thing happened to me with the fingerprint, I had it before ( for a visa visitor) and they accepted it at first but later they asked me to do it again. When I went looking for an answer I found that you have to do your fingerprint even if you have them and not expired if it is for a PR.
I will call, but it's been a while that I am trying and my call doesn't get me to speak to an agent.

So went to the doctor today, to find out that I am not the only one in the same situation! Been trying to reach IRCC since yesterday in vain, I filled a webform and still waiting, the doctor advised me to do the exam again since they can't use the ancient results to fill the IMM 1017, there is two different EME on them.
Monday I'll do the exam, and I will keep you updated. According to the doctor, they might have changed the validity of the medical exam, mine was going to expire in March 2025. The funny thing is that on my GCKEY , it shows you need a medical exam, on the tracker it shows, medical passed.
