One query - it might be my ignorance but need valuable advice.

Situation: My old passport which was from Nov 26, 2006 - Nov 25, 2016 , did not have my surname on it. It just had "xxxxx" in the given names. We then changed it completely by going through the change of name procedure through the Indian consulate in the UAE. The new passport then had the name as it is in my newly renewed passport (issued in Canada) and also the old passport on which I had travelled to Canada in 2017 (the passport with student visa).
Nowhere in any of my those previous applications to CIC did I mention of name change because I did not see it relevant considering my new passport with name change will be my new travel document for next 10 years (August 17, 2010 - August 16, 2020) and now (Oct 18, 2019 - Oct 17, 2029).

Do I have to worry about this part after the submission of application? No details of my old passport (with no surname) was mentioned anywhere in my PR application, PGWP application, Student application, and 1st tourist Visa application in 2014.

Although I do have the affidavit and old passport before name change with me
One of the registered consultant with IRCC told me that raise a webform and inform them now with the old passport (before name change) and the new one with the same reason mentioned above as I thought it was irrelevant.
Kindly advice.

Thank you for your response. I have raised a webform to IRCC and have informed them about it now along with the Affidavit and copies of old and new passport. After consulting with immigration consultant and other forums, I was advised to do so as my AOR was almost 10days back.

Its not a big issue as long as you inform IRCC.

You do not have an IME number. Instead you can take your ITA and show that to your physician.

You need an upfront exam, not a Medical Instruction Letter which is for TRVs.

you do not need an IME number if you are going for an upfront medical exam.. I did mine last week without an IME number.

Okay however in my country, medicals won't be carried out without an instruction letter. So I was wondering how to get it. Thanks.

Hi guys, another AOR.
- Stream: FSW-outland - Vietnam
- NOC: 1112 - Financial Analyst
- CRS: 475 (IELTS 9977, >3 years work experience, WES Master degree, age under 30)
- EE profile submitted: Feb 26
- ITA: Mar 04 with 471 cut-off. I feel so lucky as it was the last all-program draw till now.
- AOR: Apr 28, which is 2 days before the fee increase. Have not done Medical due to COVID 19 and uploaded LOE instead.
