Yes, the address on her medical form is same as my canadian address ..

a wierd thing happen today, my wife called the doctors office back home...she has been given next day appointment but the receptionist asked my wife to bring the address of visa office where results need to sent...

I called CIC and spoke with two different representatives...both told me the doctor should know that..applicant has nothing to do with that...

The Dr. should know where to sent to medical report!

Last month, I tracked my medical report for Work Permit visa, it was received by Ottawa, but don't know the exact address. but all the Dr. should know, because I only filled out the medical forms, there were no address.

My application of PNP under Federal Skill Work Category has been received by CIO-Sydney on March 1st 2013 but i have not received AOR as of today....

I might forget to attach form IMM5620- payment form in my application... :'( If this is the case what can i do now? Can I pay on-line now and I apply outside Canada. Or do I need to attache the scan copy of IMM 5620 by email? Your help is appreciated.

In such cases they're returning the whole application package back.
If your return address remain the same - you will get it back. Second option (if you've attached payment) - they're checking your documents.

I suppose you're in Canada now, right? There is small confusion - your VO listed as Ottawa, but applied outside Canada.

If you're in Canada now - let me correct the table accordingly.
