In Process : March 2011
In Process with date: Feb 6 2012
Requested extension of MR on March 2012, since my wife was pregnant.
Send the updated document on Sep 2012.

I extended my MR when my status changes to In Process with date, not after getting the MR.

In the ADDITIONAL FAMILY INFORMATION page, it is mentioned
Complete ALL names in English and in your native language

Do we need to fill all the names in English as well as in our native language eg. Hindi

I did my Medicals on 20th November. Everything was fine except time. Due to high volume of student visa applicants, my entire medical process took more than 5 hours to complete. The following tests were done.

1. Urinalysis (Myself, Spouse & Daughter)
2. Blood Sample (Myself & Spouse)
3. Chest X-Ray (Myself & Spouse)
4. Height & Weight Measurement (Myself, Spouse & Daughter)
5. Eye Tests (Myself, Spouse & Daughter)

I have applied for PVC and will get the same within a week. Thereafter, I will submit original passports to NDVO with other required documents.

Congratulations on completing all well. A query:

When you did your medical examination I am sure the DMP must have given you the photocopy of the first page to Medical report Section A - on that page there are 4 options, namely:
A. Findings that are unremarkable or minor conditions
B. Findings that require periodic specialist foloow-up
C. Findings that may require extensive investigations or care
D. Other conditions / disorders

Which option was checked by the DMP?
