I do not think it is lost, it just needs time to be delivered.
Anyway, "me2land" a memeber of this forum sent the medical request (scanned copy) to his wife from the united states to his country of nationality, and it was accepted by the DMP and Ottawa. He got the visas with no problems later from Buffalo.

Their DMP send their medical to Ottawa and got there 3 days before mine did. I did not send any receipts and the process went pretty smooth.
